If you want a cheap (by which I mean free) and relatively easy CAPTCHA, check out PHP CAPTCHA. I haven't used it personally, but if you were to simply copy and paste it in to a site, it might look a bit more professional and realistic than a random static image (which if anyone looked at the source code would easily spot as being fake).

Eventually, it could get into classic prove-you're-not-a-robot territory with "Compose a sonnet on <subject here>"

Part of the story is the purpose of each new captcha.   Example:
1.  To prove you're not a spambot.
2.  To prove you're actually educated about the site's topic.
3.  To make damned sure it's not a spambot, because there's personal info in here.
4-20.  Extension of the same.

Add a MSPaint webcomic with the "author's"/"sysop's" avatar smiting various forms of spammer/enemies/bots, possibly with increasingly myth-stylings.
