The thing the guy has is most definitely a siphon/pump. Not sure why it's in his mouth since it has a pump, but I don't know much about plumbing. It may be how they get the toilet initially set up.

That guy is breathing the air in the sewer because there's thick smoke from a fire.

It was an actual invention that never caught on because people would rather suffocate than breathe sewer air.

Hah, no shit? That's pretty cool actually. I'd probably choose breaking a window or blowing a hole through a wall over that, but still, that's kind of ingenious.
