Either BH has his wife completely fooled (unlikely), or she is one scared, humiliated woman.

Neither.  Unless she's hiding that fear and humiliation very very well.

3rd possibility: BH's wife is a revolting headcase just like he is. This would probably be the only thing that could adequately explain her decision to marry BH.
