This is the idea I have: A setting which is a complete inversion of a more common one - the one where dark supernatural powers from another world are secretly at work on Earth, controlling things from behind the scenes. In this story, the fantasy world is oblivious, and it's the great powers of Cold War-era Earth who are manipulating events from behind the curtain. Or, to look at it another way: It's like D&D, except when you get to fight Tharizdun it turns out that he's actually a CIA agent.

Meanwhile, the CIA was preparing a decades-long operation to destabilise and overthrow the ruling powers of Theras and replace them with US puppets. To avoid retaliation, CIA agents acted under the guise of various cults, secret societies and brotherhoods. From the point of view of Theras, some dark extradimensional entity has begun sinking its tendrils into one nation after another. They just don't know that this dark extradimensional entity is the United States government.
